Advanced registration and payment of the registration fee is required to reserve your place for the conference.
You may register online, by fax, or by postal mail. Complete instructions are included on the registration forms.

Physician: $95
Nursing and Allied Health: $50
Resident/Fellow: $25

Registration fees are fully refundable to EMC-affiliated physicians and EMC-employed healthcare professionals upon presentation of the EMC ID badge. No refund will be made if you cancel within the last 24 hours before the conference starts.

Advance registration must be accompanied by payment in full. Funds payable in U.S. dollars. All online and fax registrations must include the number of a major credit card that will be charged for the registration fees.

Mailed registrations may include payment-in-full by check made payable to Annenberg Center for Health Sciences. Purchase orders are accepted with mailed registrations.

If you wish to register multiple attendees with a single payment, please use the mail/fax form.

If you register online or by fax, please do not follow up with a mail-in registration; you may be billed twice.

confirmation / receipt

All registrations (whether online, faxed, or mailed) will be confirmed with a receipt/confirmation letter by postal mail within 10 days of receiving your registration.
